Summary:The Case Manager assesses, plans, implements and evaluates the needs of patients for discharge planning and utilization review. Discharge Planning/ Utilization Review functions are performed for patients who have Medicare, Medicaid, Managed Care, Private Pay or no insurance coverage. Discharge plans are coordinated with physicians, the multi-disciplinary team providing care for the patient, external Case Managers, the patient and significant others who have an ongoing caring relationship with the patient. The Case Manager serves as an advocate for the patient and their significant others, assisting them to navigate the health care continuum and secure services necessary to promote optimal health outcomes. The Case Manager considers clinical and financial aspects of patient care when carrying out his/her duties. The Case Manager participates in establishing standards of care and managing variances from those standards.
Requirements: Meets and maintains the minimum qualifications for the position including:
Current RN licensure for the State ofMaine.
Demonstrates clinical competencies associated with assigned areas.
Maintains working knowledge of reimbursement and regulatory issues affecting the continuing plan of care.
Skills: Basic computer knowledge.Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
