Summary:The role of the oncology patient navigator (PN) is to serve as a consistent care coordinator throughout the continuum of care assessing the physical, psychological and social needs of the newly diagnosed cancer patient.The assessment is used to plan healthcare pathways with the collaboration of the multidisciplinary team for desired outcomes achieved within effective and realistic time frames.The PN monitors the patient and triages needed services across all product or service lines while ensuring proper access to these services.In addition, s/he extends services to help the family learn care-giving skills and patient self-care skills.
Requitements: Must be anRN and hold aValid State ofMainelicense. Must be agraduate of an NLN accredited program. Must hold avalid driver''s license.This CMHC position must meet the criteria for evaluating vehicle operator suitability.It has been determined that this position requires either the operation of a system or leased vehicle or the use of a personal vehicle for business purposes on a regular basis an integral part of the job.
Skills:Cultural competency in working with underserved populations.Basic computer knowledge.Public speaking skills.The ideal candidate will have a BSN and 1-3 years case management experience accessing community resources that serve health disparate populations.The ideal candidate will have oncology experience and will demonstrate the ability to respond to financial challenges facing patients.
Oncology experience strongly preferred
Case management experience preferredEstimated Salary: $20 to $28 per hour based on qualifications.
